Resources for CST Initiative and Care Coordinators
Two full-day regional meeting/workshop/learning collaborative opportunities will be scheduled in each of the five State Department of Health Regions each year. In addition, a Care Coordinator Fundamentals 2-day workshop will be scheduled in each region.
Quick access to commonly used forms such as the Assessment Summary, Plan of Care, Team Meeting Review, Closure Surveys, and Coordinating Committee Progress Report.
The Coordinated Services Team (CST) Initiative has collaborated with the WI Department of Children and Families' training resources, including their “CANS Tool Online Training and Certification” system.
Twice annually, Coordinators from Counties and Tribes developing or sustaining CST initiatives meet for a business and networking meeting. Representation is expected from sites receiving CST grant funding.
Sites with CST initiatives are required to report data for children enrolled in the areas of education, juvenile justice involvement, living environment, and CANS-Comprehensive scores. This data is entered on a regular basis into a web-based data collection system (PPS). The Division of Mental Health and Substance Abuse Services (DMHSAS) holds periodic trainings to orient individuals to the system,
There are several annual reporting and evaluation requirements for counties and tribes receiving funding to develop or sustain Coordinated Services Team Initiatives.
This 2-day workshop is intended for care coordinators/team facilitators, team members, and family members who wish to learn the fundamentals of the Coordinated Services Team process, with an emphasis on care coordination and team facilitation responsibilities and skills.
