This report reflects the far-reaching effects of collaborative team efforts across the State by capturing the number of children and family members served. The Project Coordinator typically takes the lead in completing the Update with input from the Coordinating Committee. Results are entered and transmitted electronically via the DHS Website. Statewide and site-specific information should be shared with the Coordinating Committee.
Due Date: Results of the survey are submitted annually to the State via web-survey by Jan 30th.

Completed annually by both CST and ISP sites, as well as sites that are part of the Western Regional Partnership Grant (RPG). This system review should be completed annually by your site's Coordinating Committee, your Project Coordinator and (if applicable) with your site's CST Consultant. Results of this survey should be used by your Coordinating Committee to guide planning for the upcoming budget year. Sites that receive funding for both ISP and CST should complete one single "Annual System of Care Review" to reflect all of the children served by your ISP/CST.
Due Date:
The Annual System of Care Review is due annually (January 30th) by all ISP & CST sites. CST and RPG sites should submit their completed review to White Pine Consulting; ISP sites should submit their completed review to their State ISP Liaison (Nancy Marz or George Hulick)

Completed annually by both CST and ISP sites. Bureau of Prevention, Treatment, and Recovery (BPTR) staff send notice and forms to Project staff. The Project Coordinator typically takes the lead in completing the Work Plan with input from the Coordinating Committee. Goals and tasks in the Work Plan should reflect results of the 8 Key Components and/or Goals and Expected Outcomes Checklist.

Sites with July 1st - June 30th Contract Periods: Due May 1st (60 days prior to new contract period)
Sites with Oct 1st - Sept 30th Contract Periods: Due Aug 1st (60 days prior to new contract period)

Annually (typically Sept/Oct), the BTPR and Wisconsin Family Ties (WFT) work with CST and ISP sites to distribute surveys to families enrolled in CST/ISP. The survey is completed by the family. The Service Coordinator may offer support - answer questions, clarify, encourage completion, etc. Completed surveys are mailed directly to WFT. Site-specific and statewide information should be shared with the Coordinating Committee.

The Project Coordinator typically takes the lead with Service Coordinators to arrange survey distribution and collection details. Surveys should be provided to all service providers who are currently on teams or who have served on teams during the past year. Results should be shared with the Coordinating Committee as a system development tool.
