Coordinated Services Team Initiative - Family Team Meeting Vignettes
The following video series was developed as an educational tool for service coordinators, team facilitators, and family and team members who would like to see an example of the Coordinated Services Team (CST) Initiative family team process played out by an actual team. The situations acted out in each video are not meant to depict a specific team, child, or family experience, rather to model the types of activities and situations that family teams may experience.

Section 1: Initial Meeting with the Family (length 10:42)
This video depicts an initial meeting between a service coordinator and the family, following their enrollment in the Coordinated Services Team (CST) Initiative. In this situation, the Service Coordinator, Greg, and Family Advocate/Peer Support Specialist, Paula, are meeting with Ethan and his mother Tiffany. Highlights include: overview of the team process with the family, explanation of peer support/advocacy, initial identification of team members, and discussion of the family’s goals. |
Section 2: First Family Team Meeting (length 11:49)
This video depicts an initial CST family team meeting. In this situation, the team's Service Coordinator, Greg, facilitates an overview of the team process, and leads team members through several exercises including the Roles, Strengths, and Goals exercise, and the development of team rules. |
Section 3: The Assessment Summary of Strengths and Needs (length 6:21)
This video depicts a meeting where the family team is reviewing and discussing the Assessment Summary of Strengths and Needs. Highlights include the team's review of family and team strengths, and review and discussion of identified needs, in preparation for the development of the Plan of Care. |

This video depicts a meeting where the family team is developing a Plan of Care. Highlights include the team's discussion regarding the prioritization of needs previously identified in the Assessment Summary of Strengths and Needs; and the development of Plan of Care goals and activities related to the Mental Health domain. |
Plan of Care Follow-Up (length 2:17)
The video depicts a team meeting following the completion of the Plan of Care. The team discusses progress toward the goals and activities outlined in their plan, and makes needed adjustments. |
Section 5: Development of a Plan for Crisis (length 9:45)
In this video, the team develops a Plan for Crisis to address crisis situations that may occur at home regarding Ethan. The team discusses interventions that have worked in the past at school, home, and in the community; and brainstorms additional possible interventions and supports. |
Follow-Up Meeting: Reviewing the Plan for Crisis (length 11:06)
In this video, Tiffany (Mother) shares that there was a crisis situation at home, and that when she used the Plan for Crisis, one of the steps did not work as expected. The team reviews the Plan for Crisis and makes necessary adjustments. They also discuss distribution of the plan to law enforcement. |
Section 7: Transition Planning (length 10:42)
In this video, team members share updates regarding progress made toward goals in the Plan of Care, and discusses transition out of the formal team process. The team plans for needed ongoing support and plans for a team celebration. Also included in this video are the credits for the video series.
